Five Hills Health Region operates long term and special care homes, in all major communities in all areas of the region. These care homes have, for many years, provided a home-like atmosphere for those no longer able to completely look after their own needs.
Prior to being admitted to a long term care facility, an assessment must be completed by a Community Care Coordinator. Requests for an assessment can be made to the Access Centre administrative offices at 306-692-5696. You will be put in contact with a Community Care Coordinator for your area.
An assessment is completed using standardized guidelines where individuals are assigned a score that measures if placement is needed. The individual is then presented to a Regional Access to Care Committee to determine the eligibility criteria for admission. Once criteria is met the name is added to waiting list.
The cost paid by the client for long term care varies, and is calculated according to income, based on provincial guidelines. The rental charge does not include the cost of telephone, cable television, medication, travel, hair dressing services, clothing or personal items.
Clients frequently ask if they have to change doctors when they go into long term care. If your doctor has admitting privileges in the Five Hills Health Region, and agrees to visit the facility, the client may keep the same doctor. If not, you may have to find a physician that does. We always recommend choosing a physician who regularly visits the care home.
