The Recovery Program works with clients who are experiencing severe and/or persistent mental illness. Community Mental Health Nurses and a Social Worker work with clients in the community, providing case management, treatment and coordination of services. This includes coordination of clinical and administrative functions, assessment, counselling, planning, linkage, advocacy and monitoring.
• Acquired Brain Injury Partnership Program
This includes a recovery focus involving training in basic living skills and crisis intervention, as well as outreach to assist clients in achieving their potential for an increased level of functioning in the least restrictive manner.
Other services include, but are not limited to: medical and inpatient treatment, medication management, residential placements and monitoring, social and recreational programs, prevocational, vocational and educational programs, family supports, and connections with community agencies and services.
Services are available in rural areas through telehealth.
Referrals are made by client’s treating psychiatrist.
